In Luke's video, he just scapes the old paste off but in others, they're using isopropyl alcohol. Now thorough do I need to be in cleaning off the old paste.

I've watched Luke's video on how to do it with a 13-inch 2013 MBP as well as a couple of others like for my machine. Obviously, there's very little that I can do about the processor or the graphics, but I think reapplying the thermal paste is probably the easiest fix. The fact that it only really happens when it's hot and that the fans are running hard makes me think it's a thermal throttling issue.
